The Pathways Program is designed to provide community-based care coordination, including resource navigation, referral to services, and support for individuals and families to improve their health and social outcomes. A client is assigned a Community Health Worker (CHW) who works to understand and support their client’s priorities and experiences, working to reduce barriers, and advocating on their behalf as needed.
Pathways clients can expect to meet with their CHW at least once per month. Services can be provided at our office or in the community at a mutually agreed upon location.
Services
- Encouragement and coaching to make decisions and choices in services and referrals.
- Direct connection to various community resources (childcare, transportation, healthcare, etc.).
- Provide resources to establish healthcare coverage, family planning, prenatal and postpartum services.
- Support with development of resumes and cover letters, practicing interview skills.
- Establish a supportive network that includes community providers and other social connections.
Eligibility
- Families with children (including pregnant individuals) and survivors of domestic violence.
And either of the following:
- Have a self-identified or professionally diagnosed chronic health condition or behavioral health concern, pregnancy included.
OR
- Have other risk factors such as housing instability, recent release from jail/institution, and social service needs.
